Our office hours are normally:
Mon - Thu 7:30 AM to 4:00 PM
Fri 8:00 AM to 4:00 PM
Occasionally we may need to alter those hours.
We have a 24 hour answering service that contacts us immediately through our pagers. The physician on call will return your call as soon as possible. If after one-half hour you have not heard from the doctor, please call back.
APPOINTMENTS -MISSED APPOINTMENTS - CANCELLATION POLICY
Our office is organized around an appointment system. This enables us to work efficiently and make every attempt to reduce your waiting time. We will try to provide urgent care the same day if possible and we always make room for emergencies. Call 614-882-2349 - Option 1 to make your appointment.
As a courtesy we have added HOUSECALLS, a service that call's our patients 1-2 days in advance of their appointment as a reminder. We recognize unexpected things come up for all of us, but as a courtesy to other patients, we request that you cancel any appointment a minimum of 12-24 hours in advance. That way the open appointment can be filled with someone needing an appointment. Non-cancelled appointments will be assessed a missed appointment fee of $25.00. This fee is not covered by your insurance carriers or Medicare and will be your responsibility to pay. Our aim is to open otherwise unused appointments for our patients, not to collect missed appointment fees. Your cooperation and consideration are appreciated.
FINANCIAL SUMMARY
Our office policy is to provide you with the maximum in patient care. This can only be done with the patient and physician office working as a TEAM to help provide you with all the benefits you are entitled to under your insurance plan. Your claim will be filled with the insurance presented at the time of service by our main office.
We would ask that you pay any balance due from co-insurance, deductible, HSA or HRA accounts or non-covered services from your first billing statement.
One way to assist us is to complete the paperwork prior to your visit. Many of our patient forms can be downloaded from our website. (www.fpcw.net) Please complete and bring with you at the time of your visit.
You will incur an administrative fee for copies of your records and specific forms. We follow Ohio Revised Code 3701.74.
We do accept VISA, MASTER, & DEBIT cards.
SELF PAYING PATIENTS
If you do not have insurance at the time of your visit, we ask for payment at the time of service. We will discount your fee up to 15% if paid in full that day.
SAME DAY WELLNESS CARE & ILLNESS CARE
Unfortunately, because of your insurer's payment policy, in some cases we may have to complete your wellness care and your illness care in two separate visits. If you have a health problem you want to discuss with your doctor during your "well visit", the doctor may decide to treat that problem and ask you to schedule another appointment for your "well visit."
REFILL REQUESTS
Our Doctors give enough medication until they need to evaluate your condition again, this visit may include blood work to maintain proper dosing of that medication & its effect on your body. The easiest way to maintain your medication supply is to keep scheduled office visits so the doctors can refill your medications on time.
Please allow 24-48 hours to process request for medications. Select Option 2 and be prepared to give the name of your medication(s), with the spelling, the dosage, pharmacy name & phone number.
WORK INJURIES & AUTO ACCIDENTS
We do not care for or do paper work for injuries that are related to work. If your injuries are related to "on the job", you will need to go through your workers compensation organization for a Doctor treating workers compensation.
We will treat our established patients for automobile injuries, but payment is expected at the time of service. We will present you with a paid receipt to give to your insurance company. |
