Appointments
Your scheduled appointment time has been reserved for you. We are aware that unforeseen events may cause you to miss an appointment, but we ask for a 24-hours notice if you need to cancel your appointment. HOUSE CALLS will call 1-2 days prior to remind you of your appointment, you may select to confirm or cancel at that time. If you miss 2 or more scheduled appointments there may be an additional charge applied.
If you miss a scheduled procedure, physical or physical medicine appointment there will be an automatic charge applied.
Insurance
Our office is committed to helping you maximize your insurance benefits.
Our billing department is open from 8 a.m. to 4 p.m. M-Th.
You will be asked at every visit to show a current, valid insurance card
due to the complexities of insurance contracts and variances in individual policies.
Without your card, you will be asked to pay at the time of service.
If we have to re-file for incorrect information given, you will be charged a service charge.
Phone Options
We receive a large number of phone calls throught the day. To better serve you,
please be aware of the "options" and the time someone is available to take your call.
You may leave a message card on options 2, 4, 5, & 6.
Option 2 - Referral Desk 8 a.m. to 2 p.m.
Option 4 - Billing Department 8 a.m. to 4 p.m. M-Th
Option 5 - Appointment desk - Any time office is open
Option 6 - Nursing Staff 8 a.m. to 3 p.m.
Option 8 - Repeats the options.
Option 3 - Life Threatning Emergency ONLY - Any time office is open
